Kansas Wesleyan Sports Medicine Registration

Required documents for sports participation can be found through Healthy Roster.  Once you have been added to the team you will receive an invite to complete your paperwork.
 
If you have not received and invite, please email sports.medicine@kwu.edu, with your Name, Sport, Date of Birth,  and athletes email.  The Sports Medicine staff will get that invite out in 1 to 2 business days.  
 
All physicals are completed at school by our team at Salina Regional Orthopedics and Sports Medicine.
*If you have had recent surgery or are seeking treatment for an injury please bring surgical notes, rehab notes, any clearances, etc.
 
Students will at least receive a physical every two years while in attendance.  You may waive having a physical your 2nd and 4th year.  This form can be printed and filled out at the link below.
 
All Students must additionally complete the Insurance Waiver Portal which generally opens around June 15th.
